What is a phase gate, and who runs it?

A formal review at a phase's end, where exit-criteria evidence is tested and one decision recorded: proceed, proceed with conditions, or hold. The sponsor chairs it.

Can phases run in parallel?

Yes, by workstream, provided its entry criteria are met — cleansing during design, integration build during configuration. Dependencies cannot be parallelised away: no credible testing on an ungated build.

What happens if a phase fails its gate?
